Terms of Agreement

By submitting this form, the contact listed above, an authorized representative of this institution hereby agrees that all e-text files from the Adaptive Technology Center for New Jersey Colleges will:

a)  Be requested and used by the Institution for the sole purpose of enabling  eligible students with visual, hearing, and/or learning disabilities to meet the academic demands of the institution;

b)  Be used only for educational or education-related purposes;

c)  Not be provided to ineligible students, copied or sold.

I assure the Adaptive Technology Center that:

a)  The student has presented written evidence of an eligible disability;

b)  The studed is registered for one or more classes by this New Jersey college or university;

c)  Or is registered in an extension course offered off campus by this New Jersey college or university.
